HHSC's nursing facility directory lists 85 licensed nursing facilities in Bexar, Comal, Guadalupe, Kendall and Medina counties, with 10,338 licensed beds, as of October 22, 2026.
Nursing facilities are licensed under Texas Health and Safety Code Chapter 242 and most are also certified for Medicare and/or Medicaid, shown in the Certification column.
No. A nursing facility provides 24-hour licensed nursing care and can be paid for by Medicare for a short rehab stay or by Medicaid long-term; assisted living provides personal care without round-the-clock nursing.
